First Semester - First Quarter

Eligibility deadline - 3rd week - January - next year

Eligibility deadline June 1 (12 months after high school graduation)

________ I am enclosing my second semester/quarter grades showing at least 12 hours completed with a minimum 2.00 or better grade point average. I have enclosed a self-addressed stamped envelope. Please send my scholarship payment.

________ I do not qualify for scholarship payment. I will forfeit my scholarship.

________ My plans have changed. I will forfeit my scholarship.

Congratulations! You have been chosen to receive a Seward Community Scholarship Award. To qualify for the scholarship payment, you must complete a minimum of twelve college credit hours, and maintain a grade point average of 2.00 (on a 4.00) or better.

To collect your scholarship payment please mail a copy of your first/second semester/quarter grades with a stamped, self-addressed envelope to Lynn Schuckebier, Treasurer at the address shown above. It is your responsibility to furnish proof of eligibility to the Treasurer of Seward Community Scholarship. For your convenience we have enclosed a form with this letter. Please use this form when submitting your grades. The copy of your grades will be returned with scholarship payment.

The deadline for submitting your grades to qualify for your scholarship awards is June 1 (12 months after graduation). If you have not submitted your grades and hours to the Seward Community Scholarship Treasurer by that date, it will be assumed that you are forfeiting your scholarship. If your course of study varies from the usual semester schedule, you have one year from the beginning of your first term to qualify for scholarship payment.

It is your responsibility to notify Mr. Schluckebier to request a change in the eligibility period. We look forward to hearing from you. Thank you.
